MEDIA RELEASE FORM

(For Street Outreach, Community Events, Vlogs & Social Media)

This Media Release allows Christway Outreach (DBA “Christway”) to film, photograph, and document volunteers during community service and outreach activities. This ensures we can safely share our mission, tell stories, and highlight the work happening in the community.

Permission Granted

By participating as a volunteer with Christway, you grant Christway the unrestricted right to:

• Film and photograph you during outreach and community events
• Record video and audio for vlogs, testimonials, and recap content
• Capture behind-the-scenes moments and general volunteer activity
• Use your image, likeness, and voice in:

  • Social media posts

  • Outreach recap videos

  • Website photos and updates

  • Documentary-style footage

  • Donor communications

  • Promotional and educational materials

  • Future Christway content that highlights community impact

Usage Rights

You authorize Christway to use this content:

• In all formats (digital, print, video, photo, audio)
• On all platforms (website, social media, newsletters, etc.)
• For nonprofit and ministry purposes
• In perpetuity (no expiration)
• Without restriction on geographic location

Ownership

All photos, video, and recorded content created by Christway remain the property of Christway Outreach.

You waive any right to:

• Review or approve the final products
• Request removal after publication
• Claim ownership of any media created

Compensation

You understand that volunteer participation is not compensated, and you waive any right to financial claims, royalties, or future payments related to the use of your image or voice.

Release of Liability

You release Christway, its leadership, volunteers, and partners from any liability or claims connected to:

• The recording and use of your image
• The editing, distribution, or publication of media captured during Christway activities

Acknowledgment

By submitting the volunteer form or volunteering with Christway, you acknowledge that you have read, understand, and agree to the terms of this Media Release.
